I'm not sure if this has been reported yet, but recently I was in a UHC. I started suffocating in gravel (Normal) but then once i broke all of the gravel, I was still suffocating and I eventually died because it wouldn't stop.
The death message was "Aylinn suffocated in a wall".
This has happened to multiple people.

Not sure if you would consider it as bug or it's a general Minecraft bug, just wanted to let you know.
