Well you will need to upload this to a file viewer website, for example, you can upload it to:
Mediafire: https://www.mediafire.com
Gyazo: https://www.gyazo.com
i.mgur: https://www.imgur.com

Yeah you can use any of those, personally, I recommend Gyazo: because 1 you can install it on your computer and upload files easier, and 2 because it has a simple interface which you can understand quickly

Finally, you will need to put this form in the correct log area which is here:
