How is that possible? If you're at 1/1000 points and you lose 213 so you're on your demotion, youd gain 213, not 221. The only conceivable way is if you did 9 placement matches, lost one and got minus more than 213 and then won one and gained 221. I'm not sure if that's possible but that's my thought.

If you die with only 3 points and are sent to demotion, you will gain the points that will put you RIGHT back to either a point or two less, or more points will be gained. If I can remember, I died when I had 10 points. I understand what "logical answer" you're trying to come up with, but the only way you would gain 213 is if someone is of either the same rank or higher. Otherwise, this is possible.
