I'm not sure if this was suggested already, but it should be added, since a lot of people got banned for it, but it doesn't say it in the rules.I know they should probably understand that it's not allowed to have those kinds names, but then there's smart-asses who say it's not in the rules and therefore it's allowed.
Would be nice if the rule was added :)

That actually sounds like a pretty good idea, but I'm pretty sure the devs don't feel like coding a plugin just to satisfy people who aren't mature enough to play on their servers.
