If the number is 0, it means they have less than half a heart. Minecraft shows only half and full hearts, not smaller amounts.

he is saying that the number over his head says 0, but the player is on 20/20

This is a visual glitch, if you go up and punch em in spec their health will display the correct amount.

This is a client bug. We may be able to fix this with little hacks put everywhere but not worth the time right now.
