It's a mod called OpenEye. Basically this allows you to see why you crashed when your minecraft crashes. It records and saves the log, and next time you log on, there will be a button next to your "Mods" button ( if you're running Forge ). Once you click the button, it'll give you a link and when you open it, it'll take you to the website and tell why you crashed. Also it shows you what mods could've effected the crash and how it could be prevented, which I find pretty neat.
It is made by the same guy who made ArmorHUD, StatusEffect and DirectionHUD, bspkrs.
